We also provide Service Coordination ( SC ) , Community Transitional Services ( CTS ), Home and Community Support Services ( HCSS ), Independent Living Skills ( ILST ), Moving Assistance( MA ), Respite Services( RS ), and Wellness Counseling( WC ) under the Nursing Home Transition and Diversion ( NHTD ) waiver program. This is a Home and Community Based Services ( HCBS ) program administered by the New York State Department of Health ( DOH ) to assist individuals with disabilities and seniors toward successful inclusion in the community, especially participants living in the Nursing Home. Since no two clients are the same, Jarme provide a customized plan of care adjusted to your needs and Health condition. we work greatly with patients family and Physicians to ensure that their recovery at Home is achieved with dignity.
